Information Requested:	Manufacturer, model, serial number (if applicable) of product in need of repair or service. Purchase Order Number that the item was purchased on, and approximate date of purchase. A description of the problem or issue that needs to be resolved. A point of contact including phone number and physical location of the product in need of service.
Replacement Parts:	NWN Corporation will make available equivalent replacement parts for a product sold to the State for at least three years after a product is discontinued.
